Can it write to the patient chart by itself?

No. The draft is inert until a clinician reviews it and presses Apply.

Why is the recording limited to 28 seconds?

The speech provider's synchronous API rejects anything over 30 seconds. We stop short of that so a dictation is never lost after you have already spoken it.
